Summary

  • Active middle-ear implants (AMEIs) mechanically drive an ossicle or cochlear window. They amplify sound for the operated cochlea and require usable cochlear and neural function.

  • Consider an AMEI for selected conductive, mixed or sensorineural loss when conventional aids are unsuitable or inadequate and the cochlea can benefit from amplification; compare with bone-conduction and cochlear-implant options in the implant MDT. NHS England BCHI/MEI policy 16041/P

    Electromagnetic devices: a coil and magnet generate movement. Piezoelectric devices: a ceramic element deforms under electrical drive.

  • Advantage: an open ear canal without an occluding earmould. Disadvantages: middle-ear surgery, coupling failure and lifelong device follow-up.
